Polycystic Ovary Syndrome: Understanding Root Causes in Preconception & Postpartum Health 

Polycystic Ovary Syndrome (PCOS) is often framed as a fertility issue — but that’s only part of the story. If you’ve been told you have PCOS (or suspect you might) and you're thinking about pregnancy or trying to recover after birth, the deeper question is: What’s driving it?

Why Root Causes of PCOS and Hormone Imbalances Matter Before Pregnancy 

For many women, the first time they hear “you might have PCOS” is during a fertility work-up. But PCOS isn’t just about irregular periods or polycystic ovaries seen on ultrasound — those are signs, not causes. 

Before we begin, I want to offer hope. Even with a long history of irregular cycles or hormonal imbalance, healthy pregnancies and regular cycles are still possible. I’ve lived this myself. In my early 40s, with four children, I now have the most consistent and healthy cycles I’ve ever experienced. But in college and medical school, my periods were irregular, my relationship with food was disordered, and I had many signs of PCOS — though it was never officially diagnosed. 

After nine months without a period, I saw an OB-GYN. Though I wasn’t trying to conceive at the time, I knew I wanted children someday. I remember crying in her office, sharing my fear that I might not be able to. Years later, I ran into her while we were both working at the same clinic — I was pregnant with my first baby. It was a full-circle moment. 

Why Investigating PCOS Early Matters 

Understanding the root cause of PCOS isn’t just about symptom relief. It can: 

  • Restore regular ovulation — increasing your chances of conceiving naturally
  • Improve metabolic health — which is vital for pregnancy and your baby’s long-term well-being  
  • Reduce long-term risks — including cardiovascular issues and postpartum mood disorders 

Here are three major contributors I often see:  

1. Insulin Resistance 

One of the most common — and often overlooked — drivers of PCOS. Many women with PCOS have normal blood sugar levels, yet underlying insulin resistance that fuels high androgens like testosterone. These elevated androgens can cause irregular cycles, acne, hair loss, and weight challenges. 

📍 Insulin resistance is often missed in conventional labs, especially when fasting glucose is normal. 

💡 Tip: Ask your provider for a fasting insulin level, HOMA-IR index, or a 2-hour glucose challenge — these are more sensitive ways to detect insulin resistance. 

2. Inflammation & Stress 

Low-grade, chronic inflammation can disrupt hormonal signaling and interfere with ovulation. Common contributors include gut imbalances, food sensitivities, toxin exposure, autoimmune activity, or latent infections. Chronic stress also plays a major role — it drives up cortisol, which in turn raises insulin and androgens, leading to irregular cycles.

Aside: What About “Post-Pill PCOS”? 

Some women experience a temporary surge in androgens after stopping hormonal birth control — leading to symptoms like acne, hair loss, and delayed return of regular periods. If you didn’t have PCOS symptoms before starting the pill, this is likely a post-pill hormonal imbalance, not true PCOS. Symptoms often resolve with time, but if they persist or worsen, professional support can help.
